Debate Over New Guidelines for Breast Cancer Screening

    周一,一个有影响力的医疗专家组织发布了乳腺癌检测的新的指导方针。该指导方针来自美国预防医学工作组(U.S. Preventive Services Task Force)。该小组成员由政府委任,但是其建议是独立的。

    新的指引发表在《内科学年鉴》(Annals of Internal Medicine)上。

    其中一个最大的变化是,工作小组建议大多数50岁以下女性不要使用乳房X线照片。2002年,该组织曾建议40岁及以上女性每一到两年进行一次类似的照片检测。

    该指引还表示,50至74的女性不要像现在建议的每年进行乳房X线照片检测。专家现在建议该年龄段女性每两年进行一侧检测。

    工作小组表示,新的建议对于患乳腺癌风险增加的女性来说并不适用。

    专家还表示,没有足够的证据表明75岁或以上女性接受这种检查有利还是有害。

    但是该工作小组建议不要教给女性进行乳房自我检测。他们表示,有证据表明自我检测不会减少乳腺癌致死的风险。

    错误的检测结果——通常叫做虚假阳性——是一个问题。但是该工作小组指出另外一个问题是过度诊断。在这种情况下,医生发现或者治疗了原本不会缩减女性寿命的癌症。

    进行乳房X线照片检测的辐射也是另外一个需要考虑的因素。不过工作小组说,这个问题就比较小。

    该工作小组没有建议使用哪一种特定的乳房X线照片检测。他们表示,没有足够的证据来表明数码X涉嫌还是核磁共振成像比传统的照片检测更好。

    一些医生表示,新的指引会减少女性进行不必要的治疗的机会以及相应的危害,包括心理上的危害。但是其他医生表示,他们担心这种改变会减少检测,导致更多女性因乳腺癌死亡。

    美国癌症协会表示,他们将继续建议40岁以上女性每年进行乳房X线照片检测。但是乳腺癌专家Susan Love表示,新的指引与大部分其他国家的指引类似。
